Information You Provide to Us

Depending on how you interact with FTDS Systems, you may provide information such as:

Purchase Information: information related to purchases of FTDS products. Payment card information is processed by third-party payment processors and is not stored directly by FTDS Systems.

Success Story Submissions: information, statements, photographs, videos, or other materials you voluntarily submit to us regarding your experience with FTDS Systems.

Communications: information you provide when you contact us by email, through our website, or through customer support.

We do not intentionally request detailed medical histories, diagnoses, medication information, blood-pressure readings, blood-glucose readings, or similar medical information through our standard website forms.

11. FTDS Success Center and Member Accounts

If you voluntarily submit a Success Story, testimonial, photograph, video, or other material to FTDS Systems, we may collect the information and content you provide as part of that submission.

We may review and respond to your submission and, when appropriate permission has been obtained, use or publish the submitted story, testimonial, photograph, video, or other material in connection with FTDS Systems, including on our website or in promotional materials.

Submitting a Success Story or related material does not guarantee that it will be published or used. If we need additional permission before using particular content, we may contact you using the information you provided.

If you contact FTDS Systems by email, through our website, or through customer support, we may collect and retain the information you provide in your communication.

We may use your contact information to respond to questions, provide customer support, communicate about purchases, accounts, products, or services, and send other administrative or service-related communications.

At launch, contacting FTDS Systems, submitting a Contact form, or purchasing an FTDS product will not automatically enroll you in a general marketing newsletter solely because we have your email address. If FTDS Systems offers optional promotional or marketing communications, you may choose whether to receive them and may unsubscribe from promotional emails using the instructions provided in those communications. Opting out of promotional communications will not prevent us from sending transactional, account-related, or other necessary service communications.
